EPC Financial Policy



It is important in providing quality care and service to you, that we ensure you understand our financial policies. If you have any questions or concerns, please call or e-mail our billing department and they will gladly assist you.

  • Your insurance policy is a contract between you and your insurance carrier. We are not a party to that contract and are not aware of all of its elements. As a courtesy to you, we will bill your insurance company, however, it is your (the patient’s) responsibility to make sure that your insurance carrier remits payments and to pay any charges not covered by your carrier.
  • If payment is not remitted by the insurance carrier in a reasonable amount of time, you will be responsible for payment. Any insurance payments received after payment has been made by you will be refunded to you.
  • There is no guarantee that procedures and services will be covered by your insurance carrier. If you have a question regarding coverage, please contact your insurance company. If a procedure or service is deemed not covered, you will be responsible for any and all charges.
  • Payment is due upon receipt of bill from our office. If payment is not received after 2 consecutive billing cycles, your account may be sent to collections.
  • There is a $25.00 fee for all returned checks.

To assist in getting your claims paid in a timely manner we ask the following:

  • Insured: Please bring your insurance cards and ID and present them at check in. Please bring any co-payment or co-insurance. We accept all major credit cards and cash and checks.
  • Self Pay: If you are uninsured payment is expected in full at the time of your appointment unless prior arrangements have been made.
  • High Deductible Plans: Patients are asked to pay at least ½ of their procedure cost at time of service.

Should you need to cancel your appointment, please notify your physician's office at least 48 hours prior to you scheduled procedure date.

EPC is a surgery center. You will receive a separate bill from us. You may also receive bills from your provider, anesthesiologist and a laboratory depending on the extent of your procedure.
